Details

If we have requested for you to attend for fingerprints as part of your Standard or Enhanced DBS application, you can download the fingerprint consent form and the voluntary disclosure form (VDF) above.

The VDF is optional and can be used by applicants who have additional information to provide in relation to previous interactions they may have had with the police. This information may include cautions, fines or even incidents where no further action was taken.

If you choose to complete the voluntary disclosure form, please also complete the AF 14 fingerprint consent form.
