Use this form to apply to the Fish Health Inspectorate to be authorised to import live fish or shellfish, as required under The Aquatic Animal Health (England and Wales) Regulations 2009.

This covers imports for:

• aquaculture at an听authorised听farm听听听
• further听processing for human consumption听
• your personal collection听
• ornamental trade (susceptible听and听non-susceptible听species)听
• relaying听and depuration听
• scientific research

Before starting this听application听you should听know:听

• whether听the fish you want to import are non-susceptible or听susceptible 听species,听if your reason for importing is ornamental trade听
• your听authorisation听number - if you want to renew an existing听authorisation听
• the听site听details, including the national grid reference听
• the type and number of holding facilities听you鈥檒l听use, if听any听
• the suppliers鈥� details and听fish species 鈥� or just the supply country for听the听import of any non-susceptible听species for听ornamental trade

We aim to process your application within 15 working days. We will contact you if we need more information.听

Site inspection听

A site inspection may be needed for imports of:听
